Critical Benchmarks Months 6-9

  • Strength: 90%+ quad strength symmetry, hamstring and hip strength equal or greater
  • Hop tests: 90%+ limb symmetry on single-leg hop, triple hop, crossover hop, timed hop
  • Movement quality: No compensations during cutting, landing, change of direction
  • Psychological: ACL-RSI score >56, high confidence, low fear
  • Time: Minimum 9 months post-surgery (many protocols require 9-12+ months)
The 6-Month Trap: Many patients feel physically ready around 6 months. However, graft maturation continues through 12+ months. Rushing return to sport before 9 months significantly increases reinjury risk. Elite athletes with perfect rehab compliance and test scores still wait 9-12 months minimum.

Month-by-Month Typical Progression

Month 6:

  • Advanced plyometrics and agility
  • Controlled change of direction drills
  • Sport-specific patterns without opposition

Month 7:

  • Increased speed and complexity of movements
  • Begin baseline return-to-sport testing
  • Light non-contact practice participation (if sport allows)

Month 8:

  • High-intensity sport-specific training
  • Repeated testing to track progress
  • Full non-contact practice participation

Month 9:

  • Final return-to-sport testing battery
  • Clearance discussion with surgeon and PT
  • Limited contact practice (if cleared)
  • Psychological readiness final assessment
